Visual Editor support for list-defined references (Community Wishlist/W259)

Description

List-defined references have been supported for years in the source editor, but support is still lacking in the Visual Editor. Currently, an editor using the VE cannot in rough order of importance:

• Add a list-defined reference
• Remove a list-defined reference
• Remove the last in-line citation to a list-defined reference
• Replace a list-defined reference if it contains a template like {{Cite book}}
• Comment out an unused list-defined reference
