The U.S. Army 203rd General Hospital unit was activated on February 10, 1941, to meet the anticipated needs of an expanding armed force. As the initial events of World War 2 were taking place in Europe, it served primarily as a training unit for army medical technicians. At this early stage of its existence, the 203rd was stationed at Fort Lewis, Washington, as a subsidiary of the base Station Hospital there.

In 1942, the mission of the 203rd changed, and it became an independent, cohesive General Hospital unit, training its own personnel, and preparing them to serve together in "overseas operations" under combat conditions, in any World War 2 theater of war. In December of 1943, around 500 strong, the unit left Washington state for an unknown east coast Port of Embarkation. They then sailed for Great Britain, and learned that they were destined to take part in the great preparations being made there, for the invasion of Nazi-occupied France.

The 203rd initially served in England, and treated troops wounded in the D-Day invasion of Normandy, in June, 1944. In July of that year, they followed the invasion troops to Northern France, landing on Utah Beach, and serving in the field until Paris was liberated in August,1944. At that point, they were transferred to the Parisian region, where they operated the largest hospital establishment in the European Theater of Operations until July of 1945, treating over 65,000 patients during their 11 months of work there.
