How does the affiliate make decisions? How are decisions communicated? mostly informal, there is no board (not yet, it's still in discussion)

Affiliate health and resilience
How many people participate in the decision-making process in the affiliate? N/A (at least one of the primary and secondary contacts and the most involved person in the community)
Has there been any significant changes in the level of the affiliate membership body (e.g. increase/decrease of number of dues-paying members)? there is no membership, everyone can join freely, the listed people on Wikisource_Community_User_Group#Interested_users went from 157 to 169 (but some are inactive, hte list may need some cleaning)

Diversity balance (especially gender)
What is the diversity level of the affiliate leadership and membership body? The primary contact is an European man, the secondary contact is an Arabic woman.
How is the affiliate leadership encouraging gender diversity in the decision-making process(es) of the affiliate? N/A
Universal Code of Conduct compliance
How does the affiliate leadership ensure, enforce, and evaluate compliance with the Universal Code of Conduct obligations? The UCoC is reminded whenever there is a meeting or event. A person responsible as a point of contact is designated if needed.

How many affiliate members are not active contributors to the Wikimedia projects? Most members are part of the Wikisource community (to various level of activity).
How many affiliate members are active organizers (of projects, events, conferences, initiatives, etc) within the Wikimedia movement? Maybe 20 ?
How many people have participated in affiliate projects, events, conferences, and/or initiatives in the past year? 100+ (thanks to the Wikisource conference)
