Help with this form
This form should be completed by grantees who are receiving next installments or additional payments on a grant. These grantees have already completed the information needed to process the first payment of a grant, and so only need to confirm that certain information has not changed or need provide updated information. The grants administrator completes the first part of this form (questions 1, 2, 3, 4, 5 and 6), while the grantee completes the second part of the form (questions 7, 8, 9 and 10). This form is the same for all types of grantees.

Information completed by the grants administrator
The grants administrator will list the name of the grant and the grantee and will also describe any conditions that need to be met in order for a next installment to be sent. These conditions will be outlined in each grant agreement, but may include things like submitting a midpoint report on time or making sufficient progress on a grant. The grants administrator will also verify if these conditions have yet been met and will list when the installment will should be sent. Finally, the grants administrator will list the total amount approved for the grant, the total amount already sent to the grantee, and the total amount to be sent with this installment. If any of this information does not appear correct, please let the grants administrator know right away so that we can explain or correct the information.
Information completed by the grantee
- Bank information: Has your bank information changed since you completed your bank information form? It is important to inform us of any changes to your account, so we may be sure to send you the funds without delays.
- Grantee information: Has access to your bank account changed since you completed your grantee information form? For example, has anyone's bank account access been added or removed since you received your first payment?
- Grant agreement: Are you currently fulfilling the requirements of your grant agreement? In order to receive more funds, you must be meeting your current agreement.
- Additional information: Is there anything else you need to tell us about your grant before we send your next installment? Please let us know if there is anything else that we need to know before we send you the funds. For example, has the status of your project changed? Has there been a change to the amount we need to send? Your grant agreement may require that you send us certain information about your grant.
